Referrals

Send us a referral

We respond to every referral within one business day with a clear answer on capacity — and faster for hospital discharges, because a discharge date does not wait for a callback. Complex and high intensity referrals are welcome; they are what we are built for.

Urgent discharge?

Call us rather than using the form. We can attend a discharge planning meeting, take clinical handover directly, and start interim supports while a plan review is processed.

What happens next

From referral to care

  1. 1

    You send the referral

Use the form below, email it, or call us. Give us as much clinical detail as you have — it is what lets us answer properly the first time.

  2. 2

We respond within one business day

A clear yes or no on capacity, not a maybe. For urgent hospital discharges we come back to you the same day wherever we can.

  3. 3

    Nurse assessment

Where clinical or high intensity care is involved, a registered nurse completes an assessment — on the ward, at home, or online.

  4. 4

    Supports start

Care plan written, workers trained and signed off, roster confirmed. For complex care, competency sign-off happens before the first shift.
