Expertise, led with kindness

Coming home from hospital?
We attend discharge planning meetings, take clinical handover directly and can start interim supports while a plan review is processed.
Hospital discharge supportWhere we specialise
Most NDIS providers can help with the everyday. Far fewer can deliver nursing care, high intensity supports or compliant behaviour support implementation. This is the work we built Expert 2Care to do.

Everyday supports
Clinical care rarely stands alone. We deliver the daily supports too, so your care does not have to be split across four providers who never speak to each other.
Support to get out, take part and stay connected to the things and people you care about.
Practical help keeping your home clean, safe and running, so your energy goes where you want it.
Support with everyday living, including in shared and supported independent living arrangements.
Building the practical skills that make independence real, at whatever pace works for you.
Help navigating the NDIS and the big transitions, so decisions get made with information rather than pressure.
Support to find work that fits and to keep it once you have it.
Why Expert 2Care
Expertise and warmth are not a trade-off. Our whole model assumes you can have both.

Registered nurses hold oversight of every clinical support we deliver — assessment, care planning, delegation and supervision. Not a coordinator signing off on care they cannot assess.
PEG and enteral feeding, complex bowel care, catheter support, complex wound management, restrictive practice reporting. These are the referrals other providers decline, and they are the ones we are built for.
You should not have to re-explain your care every week. We roster for continuity, which matters most where workers are trained and signed off for your individual needs.
Every enquiry and referral gets a clear yes or no on capacity within one business day — faster for hospital discharges, because a discharge date does not wait for a callback.
Our values
Values are only useful if they tell you what someone will actually do. These are ours.
Every background, culture, faith, gender identity and sexuality — matched on language and culture where we can.
Personal care is intimate. We move at your pace, and never treat a person as a task.
Your goals, your choices. We explain things plainly so you can make decisions about your own support.
We keep looking for better ways to support you — and only change how we work once we know it is safe.
How it works
No drawn-out intake process. Four steps, and you know where you stand at each one.
Call, email or send a referral. Tell us what supports are needed and when. We will tell you straight away whether we have capacity.
We meet you at home, at the hospital, or online. Where clinical care is involved, a registered nurse completes the assessment and identifies the real risks.
We write the plan in plain language, agree it with you, and match you with workers trained for your needs — including competency sign-off for high intensity supports.
Supports start on the agreed schedule. We review on a set cycle and whenever something changes, and escalate to your treating team early.
How it's funded
NDIS funding language is needlessly confusing. Here is the short version — and if your plan does not seem to cover what you need, tell us and we will help you work it out.
Day-to-day supports: personal care, high intensity daily personal activities, household tasks, shared living and community participation.
Supports that build capability: nursing assessments, life skills development, employment support and transition planning.
As a registered NDIS provider we can support participants whose plans are NDIA-managed, plan-managed or self-managed.
Yes. Expert 2Care is a registered NDIS provider, which means we are audited against the NDIS Practice Standards and bound by the NDIS Code of Conduct. Registered providers can support participants whose plans are NDIA-managed, plan-managed or self-managed.
We support participants across the Melbourne metropolitan area, including the CBD and inner suburbs, the northern, western, eastern and south-eastern suburbs, and the Bayside and Peninsula areas. If you are outside these areas, call us — we will tell you honestly whether we can help.
Yes, and it is what we specialise in. We deliver supports including PEG and enteral feeding, complex bowel care, catheter support, subcutaneous injections, complex wound care, dysphagia and mealtime management, and epilepsy support. Workers are trained against the NDIS high intensity support skills descriptors and work under registered nurse supervision.
We respond to every enquiry and referral within one business day with a clear answer on capacity. For hospital discharges we move faster, because discharge dates do not wait. Straightforward supports can often start within a week; high intensity supports take a little longer because worker training and competency sign-off must be completed first.
Yes. We work directly with hospital discharge planners, social workers and allied health teams, and can attend discharge planning meetings in person or online. We can also provide interim supports while a plan review or change of circumstances is being processed, so a participant is not stuck in hospital waiting on paperwork.
No. We are an implementing provider, which means we deliver behaviour support plans written by a registered specialist behaviour support practitioner. We train our workers in your specific plan, collect the behaviour data the plan relies on, and meet the recording and reporting requirements where regulated restrictive practices are in place.
That is the aim, and we build rosters around it. A small consistent team means you are not re-explaining your care every week, and it is safer — particularly for clinical and high intensity supports, where workers are trained and signed off for your individual needs.
Tell us, and we will act on it. You can raise feedback or a complaint by phone, email or through our feedback form, and you can do it anonymously or through someone acting on your behalf. You can also go straight to the NDIS Quality and Safeguards Commission on 1800 035 544 at any time. Using our process does not affect your right to contact the Commission.
